     因為我有在茂盛醫院看胎兒心臟超音波，醫院裡有三位超音波技術員，每週我只去一次，她們都很認真在旁邊看我做超音波，她們常會問一個問題，為什麼我可以找到需要的切面，這個問題也許困擾很多人，其實答案只有一個，在做超音波以前需要先讀書，腦中先要有影像及相關的解剖學知識，讀完書再看別人做才看得懂，看懂以後再自己練習做做看，做做看的時候盡量選擇肚皮不能太厚的 case ，假以時日，自然就會了，當你超音波會看以後再把書拿出來再看一遍，這時候你會發現書上的每個字都是作者的經驗，當你完全懂以後，接下來就是接受各種case的考驗，沒有人敢說自己什麼都會看，這時候你遇到的case 就是你的老師，如果有相關的醫學會議，一定要去參加，因為你需要去聽去看別人的優點，講者10 分鐘的內容也許是他花10 年才領悟到的道理，聽演講是最快速的學習方法，但是不紥實，回家以後還要再看資料把內容消化，否則別人的東西還是別人的，自己還是不懂；最近因為複習胎兒腦部超音波，所以做了一點筆記，分享給有興趣的人。

1. 小腦之間的inferior vermis 在 19 weeks 才發育好

2. Corpus callosum 在20 週才發育好

3. Transcerebellar plane 看到的 vermis 是 superior vermis

4. Corpus callosum 發育次序

   genu

   body

   splenium

   rostrum （continuous with the genu extending inferiorly and then posteriorly）

5. 12 週以前受傷造成corpus callosum complete agenesis，12週以後是partial agenesis （dysgenesis）

6. 有看到 CSP 一定有corpus callosum 的 genu 部分，也可以排除 lobar holoprosencephaly

7. 沒看到 CSP 也有可能有完整的 corpus callosum，此時有可能是 septo-optic dysplasia or lobar holoprosencephaly

8. septo-optic dysplasia的症狀 variation 很大，有可能 blindness to normal

9. The fornices of the hypothalamus just below the level of the CSP

10. Cavum septum vergae is a posterior extension of the cavum septum pellucidum

11. Coronal plane : 在 falx 與 corpus callosum 之間有白色的區域（或間隔）是血流區

12. Skull 是在12 週時 ossification

13. Transcerebellar plane 的 landmarks

    cerebellar hemispheres

    superior vermis

    cisterna magna

    cavum septum pellusidum

14. Transventricular plane 的 landmarks

    CSP and frontal horns

    atrium of the lateral ventricle containing the choroid plexus

    可以看到 parieto- occipital fissure on the medial aspect of the hemisphere

15. Transthalamic plane（BPD plane） 的 landmarks

    CSP and thalami

    Sylvian fissure

16. Cisterna magna size 4 ~ 10 mm

17. 在 Cistena magna 有時候會看到 arachnoid septations

18. 胎兒腦部超音波需要的切面

   Sagittal plane 3 views

   Coronal plane 5 views

   Axial plane 3 views

19. 要看 corpus callosum 周圍的血流， 需要把 flow 的velocity調低到 8~12 cm/sec ，同時需要把flow gain 調高。
